
Transition & Diversion Services
At Northern Nevada Center for Independent Living, we believe that independence is a lifelong journey. Our Transition & Diversion Services are designed to support people with disabilities as they take the next step toward greater self-sufficiency and community living — no matter their age or stage in life.
Transition Services: We help people with disabilities move from institutional settings (such as nursing homes or rehab facilities) into independent, community-based living. Our team provides guidance on housing, personal care services, benefits, and resources to ensure a smooth transition.
Diversion Services: For those at risk of institutionalization, we offer support to help them remain safely and independently in their homes. This includes access to home modifications, assistive technology, in-home services, and advocacy to ensure continued community living.
Unlike other programs, our consumers NEVER “age out” of the support we provide. We are committed to being a lifelong resource, offering assistance at every phase of life—from young adults seeking independence to older adults maintaining their autonomy.
